Employees must complete the Safety Checklist for Telework at Alternate Worksite Employee Certification to verify that their home work environments meet government workplace safety standards.

The following checklist is designed to access the overall safety of the alternate worksite. Each participant should read and complete the self certification safety checklist. Upon completion the checklist should be signed and dated by the participating employee and immediate supervisor.
